LOCAL CARE TEAM
Coordinated, Interagency Support for Your Child
The Local Care Team provides FREE problem solving for individual and family needs. We help families who have children with intensive needs access resources within the community. We are committed to helping you, even when you feel like you have nowhere to turn.
REFERRALS TO THE LOCAL CARE TEAM
There are multiple ways to access support from the Local Care Team.
We offer the following referral options:
Ask your school Guidance Counselor if a referral to the Local Care Team would be beneficial to you and your child.
LOCAL CARE TEAM MEMBER AGENCIES
The Local Care Team is made up of local agencies and non-profits who work together to help you and your child. This coordinated, collaborative approach is highly effective. Each of our member agencies is ready and willing to help based on your child's unique needs.
